Below, you will find a list of forms which you can download to your computer. These forms can then be printed out, filled out, then mailed to the address listed next to the link.

These forms are compatible with Microsoft Word. Other programs may be able to open them, however. If you cannot download the forms, contact the city for the forms that you need.

Identity Theft / Valid Identification
In order to combat identification fraud and in accordance with an Executive Order by Governor McGreevey, the New Jersey State Registrar has implemented procedures for obtaining certified copies of birth , death marriage and domestic partnership records.

The following individuals are permitted to request a record by mail or in person: Subject, Subject's parent, Legal guardian, Legal representation, Subject's spouse, Subject's child, Subject's grandchild, Subject's sibling, Commissioner.

See Exhibit 2 for other circumstances and individuals who may obtain records.

  1. An application must be completed in full and accompanied by the appropriate identification described below. Be sure to follow mailing instructions
  2. Acceptable Forms of Identification
    • A valid photo driver's license or photo non-driver's license.
    • A photo ID and an alternate form of ID with address.
                                        or
    • Two alternate forms of ID with address.

Alternate Forms of I.D:

  • Non-photo driver's license
  • Vehicle Registration
  • Insurance card (Auto insurance)
  • Voter Registration
  • Passport
  • Green Card
  • County ID
  • School ID
  • Utility bill (Gas, Electric, Phone, Cable)

Birth records may be picked up at the City Clerk's Office at City Hall, Somers Point only on Mondays, Wednesdays and Fridays from 9:00 AM until 12:00 Noon . Death records and marriage certificates may be obtained in person from 9:00 AM to 5:00 PM . All records are now $10.00 per copy in accordance with Ordinance No. 13 of 2004 of the City of Somers Point effective July 1, 2004 . We accept personal checks or cash when paid in person.

Written requests must be accompanied by photocopies of the requested I.D. as listed above. A self-addressed, stamped envelope (please send a #10 envelope 4"x 9 1/2") addressed to the same address as is on your ID should be provided with your request. Check payments are acceptable in state, however, out-of-state requests must be paid by money order made payable to the City of Somers Point. Mail requests are processed and mailed the same day.

OTHER SITUATIONS FOR OBTAINING A BIRTH RECORD

  1. The Mother of the child is under 18. The Mother's mother, (Grandmother), may obtain a copy of the birth certificate, provided the required application and identification is supplied.
  2. A stepmother may obtain a copy of her stepchild's birth certificate if authorized by the father with documentary proof that the stepmother and father are married, provided the required application and identification is supplied.
  3. A "significant other" may be authorized in writing by the other party to obtain a certificate provided the application and required identification is supplied.
  4. If a brother or sister is picking up a birth record for a sibling, they must bring their own birth record with parent's names to prove relationship to the sibling.

GRANDPARENTS CANNOT GET A COPYOF GRANDCHILD'S BIRTH CERTIFICATE EXCEPT AS FOLLOWS:

Grandparents without custody must have the written authorization of the parent in order to obtain a copy of a grandchild's birth certificate.

 

The procedure is for the parent to supply all the information regarding the birth in writing.

1. Child's Full Name as it appears on certificate

2. Mother's Full Maiden Name as it appears on certificate

3. Father's Full Name as it appears on certificate

4. Date of Birth of Child

5. Place of Birth of Child

 

The name and address of the person authorized to pick up the certificate, together with a photocopy of a photo identification with signature for comparison with the authorization. The Person picking up the certificate must also supply the required identification and complete the application upon applying for the record.

A grandparent with custody may secure a birth certificate upon presenting a copy of the custody papers and completing the required application with all birth information and supply the required identification for the applicant.

 

OTHER SITUATION FOR OBTAINING A DEATH RECORD

1. A former husband may obtain a copy of the death certificate of the deceased former wife who had custody of the children of the marriage provided proof is submitted that custody of the children has now been granted to the former husband who is the natural father.

The application form for a Vital Record is a three-part form and only the Applicant's information and the section regarding the record you wish to obtain should be completed. We are not set up to do certification copies at this time. These copies are issued without a signature and raised seal and are for information purposes only. CERTIFICATE OF PARENTAGE / DENIAL OF PATERNITY
The following forms deal with Certificate of Parentage and Denial of Paternity. These forms enable the father of a child who has not signed the birth record at the hospital to add his name to the birth record and establish paternity. By establishing paternity in this way, there is no necessity to go to court.

 

If the mother is legally married, even if separated from her current spouse, a Denial of Paternity must be completed by herself and her legal spouse in order that a Certificate of Parentage may be completed by the mother and the biological father.

If you are completing these forms prior to coming to our office, please do NOT fill in the child's information at the top. We will type that in when you bring it to our office. Also, there can be no cross-outs, whiteouts or erasures. If so, you must complete the form over again.

If these forms are not completed at the hospital when the child is born, there is a $25.00 fee to the City of Somers Point for completing them after the birth at our office. This fee includes notary fee and the correction to add the father.

We prefer to do Certificate of Parentage and accompanying forms on Tuesday or Thursday afternoons between 2:00 PM and 4:30 PM . These forms must be notarized so you will have to produce photo identification with a signature. DOWNLOAD NOW
